What's a Japanese arcade like?

I've never been to a Japanese arcade.  What is it like inside one of the nicer ones?  What games are available to play nowadays?  Are there any classics that are always available?  I'm mostly interested in the STGs.

I picture smoke infested dimly lit places, not unlike casino's Stateside.

I imagine they are ultra sterile unlike the greasy pizza place arcades of yore here in the states.
